In extenuating circumstances, practitioners may be required under law to share information to external providers. This includes records being subpoenaed by the court, if there is an imminent risk of harm to self or others, and under mandatory reporting of suspected harm to a child (Children and Young Persons (Care and Protection) Act 1998).
Your information will not be used or disclosed for any purpose other than the one it was collected for.
